Just give us a call or request an appointment online. After scheduling, you’ll receive a confirming text and email before your visit.
Please bring your ID, insurance card, your medication list, and a referral if your insurance requires one.
You’ll receive the paperwork via email after you schedule your appointment. If you can fill it out and email it back to us ahead of time, that’s wonderful! If not, no worries—you can complete it in the office.
We are located at Frank Lloyd Wright and Via Linda, in the plaza with the VA (Veteran Affairs).
Please park in front of the VA. You’ll find our office tucked in the back left corner of the plaza.
